Dear Sir/Madam, or by name (if you know it) or To Whom it May Concern,
I would like to tender my resignation for the position of (name of position/job title).
In accordance with my terms and conditions, I will be giving you (how much notice – see what it says in your contract of employment) and my last working day will be (work this out from your notice period).
I have enjoyed my time working for (name of company) and feel that I have learnt some valuable tools of work and feel that I have also brought some good experiences to the company. I now feel that it is time for me to move on and continue my career path in a different environment.

Please could you forward my final monies and P45 to my above address (ensuring you have laid out your letter with your address above). I understand that my final monies will include any work done in my last month and any holiday pay that I am due.
