Dear Mrs Smith

Please accept this letter as notification that I wish to resign from my position as Administrative Assistant within ABC Solutions, with effect from the date above.

It is with a heavy heart that I leave the position which I have assumed for the past eight years, but due to on-going ill health, I find it is no longer possible to continue my duties with the company.

I believe I am required to give you four weeks’ notice of my intentions; therefore my date on which I depart will be (insert date). As I am currently on sick leave, it is my understanding that I am not eligible to take my accrued holiday entitlement. Please advise me if this will be paid to me and, if so, when.
